Fill out the Change in certain particulars form (Form 3) to tell us of a change of premises address or staff member.

Fill out the Application for mutual recognition of a tattooist licence (Form 6) if you have an interstate licence and need a licence in Queensland.
This is a requirement under the Mutual Recognition (Queensland) Act 1992.
